The Covid-19 vaccine form to fill is here

PETALING JAYA: The health ministry has published its Covid-19 vaccination consent form which includes a clause stating signatories understand they are responsible for any possible risks.

The form was uploaded on the Penang Health Department’s website.

It requires signatories to declare if they have suffered any side effects from previous vaccinations, if they have any allergies, if they are or planning to get pregnant and if they are currently breastfeeding.

It also states that by signing the consent form, signatories understand the following:

  1. The Covid-19 vaccine may lead to side effects as stated in the Covid-19 Vaccine Information for Recipients.
  2.  The signatory is responsible for any possible risks because the benefits far outweigh the side effects.
  3.  The vaccine does not guarantee the signatory will not be infected with Covid-19 in the future.
  4. The signatory consents to receiving the vaccine voluntarily to complete the vaccination as scheduled.

At the end of the form, signatories are required to write their name and identification numbers, including if they are signing the form for those under their care.

The first phase of vaccinations, from February to April, is for the 500,000 frontliners who are directly involved in the fight against Covid-19.

The second phase, from April to August, will target the high-risk group, while the third phase, from May to February 2022, will involve those aged 18 and above.

The registration process for those who wish to receive the Covid-19 vaccine is set to start next month.-FMT
